Before Your First Appointment
For your first appointment please fill out an informational packet which you can download here. This form will go over your dental history release of information, our financial policy, medical history and a patient financial agreement.
Please try to get to the office at least 15 minutes early so that any other paper work is taken care of and so that our team can get you properly setup. Also please come with with your driver’s license and insurance card when you check-in at the reception desk.
If you have dental insurance then please read our information about how we handle insurance, what insurance companies we work with and any other important information. Just a heads up that you may have to pay a copay at the office, or your dentist might bill you the balance after your insurance pays them.

Dr. Nathan Jang takes a personal approach in patient patient care because the needs of each patient are unique. Whether the patient needs restoration, cosmetics, or dealing with dental emergencies, he wants to make sure the patient understands all aspects of his work from diagnosis to treatment. That is why he uses the newest technologies such as cameras, scanners and radiographs not only to treat dental diseases but to educate the patients.
Dr. Jang understands that dentistry is a constantly evolving field. That is why he is dedicated to incorporating innovations into his dentistry. When he is not busy treating his patients, he takes continuing education courses that allow him to stay on top of the latest developments in dentistry. His scope of dental expertise encompasses such procedures as teeth whitening, tooth extractions, root canals, restorative crown and bridge treatments, removable dentures, and dental implants.
